What you'll do
- Conduct nurse assessments and build individualized care plans
- Provide oversight, coaching, and support to caregiving staff
- Coordinate with families and medical providers
- Adjust care plans as client needs change and follow through
Minimum qualifications
- Active New Jersey RN license
- Excellent clinical judgment and communication
- Commitment to responsible, person-centered care
- Able to perform the essential functions of the role with or without reasonable accommodation
Licensing and certification
- Active New Jersey Registered Nurse (RN) license
Preferred qualifications
- Home care or community-based nursing experience
- Experience mentoring or supervising care staff
Schedule expectations
Primarily weekday hours with a rotating on-call schedule shared across the clinical team.
